ProHosting Sales Agent Contract

I, ___________________________, have carefully read and agree to the Sales Agent program rules as outlined on the ProHosting Web Site. I fully understand and accept the circumstances by which I will receive a sales commission and comprehend the applicable effective dates of when the commissions will begin on each applicable referral. I also fully understand the conditions under which commissions are not earned or the various situations under which commissions can be forfeited or will require repayment due to account cancellations etc.

I full understand that I am not an official representative or employee of ProHosting and that this program operates under an independent contractor structure. As such, I acknowledge that my sales and referral activities will be done at my own convenience, at my own expense, using my own equipment, and at my own location(s). In accordance with I.R.S. requirements, I understand that if my earned commissions meet and/or exceed $600.00 in a calendar year, I will receive an I.R.S. form 1099 from ProHosting stating the earned commissions from my Sales Agent activities.

I accept the program requirement of a $50 USD activation or set-up fee and fully understand that this fee is non-refundable and must be paid by negotiable check or money order before my account is activated.
